Arizona Care Network’s network of high-quality providers collaborate for your best care. Our Care Coordination staff become part of your healthcare team and provide services to help with challenges that impact your health. Together with your provider, ACN can improve your health and deliver a better healthcare experience. Best of all, there’s never a bill for Care Coordination services.
ACN is governed by physicians so decisions are made by those who know healthcare best. Our population health tools, resources and personal support help you save on administrative costs and succeed in the new Medicare environment. Broad payer contracts unlock your access to new patient populations.
ACN is the value-based provider network for Dignity Health and Abrazo Health, plus hundreds of independent providers Valley-wide and the Phoenix Children’s Care Network. This creates a high performing network with a proven record of success and offers a wide variety of solutions for your employee benefits.
Arizona Care Network (ACN) is a group of doctors and care facilities who work together to improve healthcare and reduce costs by actively coordinating care for our patients.
We are 5,500 doctors in 1,800 care locations, all committed to care that meets ACN’s high standards for quality and outcomes. Our services help doctors coordinate your care, even if you see multiple providers. This leads to better outcomes.
We know you have many choices and we must earn your loyalty. We’ll help remove some of the headaches of healthcare. Our Care Coordination team can guide you through any personal roadblocks to achieving your best health. They are an important part of your healthcare team. Our online search tool can help you find the right in-network provider for your needs.
Consumer experts agree, when you want to save money on healthcare, choose a provider who is in your network. And with ACN, your providers work as a team to manage your care efficiently and reduce costly duplication of services.
Because our providers need to be healthy themselves to deliver good healthcare, we support them with the tools and resources they need to succeed in the modern healthcare environment.
We had a great time facing off in ACN's "Lord of the Ping" tournament on Monday afternoon. Our team competed in novice and advanced brackets, and also had the opportunity to participate by filling out a bracket to predict the champion in each category.
It was an intense set of games as they battled it out, but Dan LeClair (advanced) and Tannaz Farahani (novice) took home the gold in their designated tournaments. Todd Ricotta, our CEO, had the most accurate bracket predictions. We are already looking forward to our next "Lord of the Ping" tournament.🏓
